In Docketwise, you can use our Custom Intakes option to:
Build unlimited questionnaire templates for different use cases
Ask anything you want by including any combination of questions:
Docketwise's pre-made questions, for which responses will be automatically saved to a contact and/or
Craft your own Custom Questions
Organize Custom Questions into Custom Tabs
Include document upload requests with upload buttons for your client (learn more here)
Share easily via Email, SMS Text, Shareable link, and Portal invitation
Translate the Intakes into Spanish, Portuguese, French, Hindi, Russian, Mandarin Chinese, Arabic, Korean, Turkish, and Haitian Creole. (Only available for pre-made questions and not Custom Questions)

Creating a New Custom Intake Template
To get started building your Custom Intake, you will need to follow the steps below:
Navigate to Settings > Custom Intakes
Click the + Create Intake button
Name your Custom Intake template
Click the Create Intake Template โ button
Editing a Custom Intake Template
In order to make edits to an already-existing Custom Intake Template, you will need to follow the steps below:
Navigate to Settings > Custom Intakes
Click on the title of the Custom Intake Template that you would like to edit
Adding Questions to a Custom Intake Template
Once you have made a Custom Intake Template, you are able to ask any questions you want by utilizing our pre-made questions and Custom Questions!
Adding Docketwise's Pre-Made Questions
When building a Custom Intake Template, you can choose from any of Docketwise's pre-made questions, for which the responses will be automatically saved to the contact and thereafter synced to the contact's other forms in Docketwise; these questions can be found under General, Immigration, Family, and Work & School sections.
After finding the question that you would like to add, simply click the slider bar to active and the question will be added to your Custom Intake!
Creating Custom Questions and Document Requests
In order to create Custom Questions and Document Requests, you will need to follow the steps below:
Navigate to the Custom section of the Custom Intake set-up
Select whether you want to question to:
Provide a space for a response with a short answer
Provide a space for a response with a long answer
Be a Document Request
Type out the Custom Question or Document Request
Click the Add Question button
Text Field (short answer):
With the Text Field (short answer), you can ask a question that would elicit a fairly short response. The text field only accommodates one-line responses (or approximately 75 characters).
Text Area (long answer):
With the Text Area (long answer), you can ask a question that would elicit a longer-form response. The text field accommodates multiple lines of text and can be expanded as necessary.
Document Request:
With Document Requests, clients (or other invitees) may upload multiple files for each Document Request question. Files uploaded will be saved in Docketwise under the Contact and Matter (if selected) to which the Custom Intake is saved.
Adding Custom Questions and Document Requests
After creating a Custom Question or Document Request, simply click the slider bar to activate and the question will be added to your Custom Intake from within the Custom section!
Adding and Using Custom Tabs
Adding Custom Tabs
In order to create Custom Tabs for your Custom Questions and Document Requests, you will need to follow the steps below:
Navigate to the Custom section of the Custom Intake set-up
Select the checkbox for Add custom tabs
Name your new Custom Tab
Click the Add Custom Tab button
Using Custom Tabs
In order to use Custom Tabs, you will first need to add a Custom Question or Document Request to your Custom Intake Template. After adding the Custom Question or Document Request, you simply drag the question to the desired tab.
Sharing/Sending a Custom Intake
In order to share a Custom Intake with a contact, you will first need to create the Custom Intake as a Smart Form by following the steps below:
Click the Create New โผ button from your Docketwise dashboard
Select Form
Select your Custom Intake Template under the option for Forms that you would like to prepare
Indicate the client associated with forms
Indicate the matter associated with the forms (optional)
Click the Create Form button
After creating the Custom Intake as a Smart Form, it can now be shared with the client via the following methods:
Importing a Pre-made Custom Intake
We have pre-made custom intakes that you can use to get started with the feature. We have from Basic Intakes to AoS document requests.
From the Custom Intake page click on Import Intake.
In the window that opens choose the intake that you want by clicking on Import.
Once imported you can edit the Intake using the pencil icon and share the intake through a Smart Form.
