Using Docketwise, you can quickly and easily prepare form ETA-9089 along with accompanying appendices and the final determination.

Below, we will go over how these are added:

Appendix A: To add Appendix A, you will need to answer Yes to: "Has a completed Appendix A identifying 'Beneficiary' being sponsored for permanent employment by 'Petitioner' been attached?"

Appendix B: To add Appendix B, you will need to answer Yes to both: "Will work be performed in geographic areas other than the one identified in Section F.a" and "Is a completed Appendix B attached?"

Appendix D: To add Appendix D, you will need to choose the option College or university teacher for "Occupation type that best describes where work will be performed".

Final Determination: To add the Final Determination, you will need to answer Yes to: "Is 'Beneficiary' submitting Form I-140 for a Schedule A or National Interest Waiver together with Form ETA-9089?"
